Patient Criteria

💙 Ages: 1 year and older

💙 Resident of DeSoto County

💙 Working at least 24 hrs per week without insurance available through employer

💙 Student taking at least 12 hours per semester

💙 Primary caregiver for children living in home

Patients will be seen by appointment only.

Non-Emergency Diagnosis

Diagnosis and treatment of acute disorders like infections, viruses, rashes, bronchitis, minor injuries, and others.

Primary Care Services

Management of chronic conditions like diabetes, hypertension, asthma, and others.

Other Services Provided

​Limited basic laboratory tests will be done onsite (more complex diagnostic work referred out)

• Well child visits/school physicals

• Prenatal and newborn care

• Employment physicals

• Integrated behavioral health screening and service

• Prescription assistance through MAP Program referral and local $4.00 monthly generic programs (no narcotic medications) Limited samples due to long term expense of costly brands

• Wellness and nutrition education

• Prayer and spiritual assistance

• Referral of and connection to an array of social services and community agencies/support
